Click here to print the 2010 CMRC Parental/Minor Participant Waiver.
The form (2 pages) can be printed off, filled in, signed and mailed to the ADRA office (Nottingham P.O., Box 79166, Sherwood Park, AB T8A 5S3) as long as it will be received before the first event attended or it can be given to the staff in the race office at the first event attended.
